Hydroxyapatite for sensitive teeth
Dentin hypersensitivity often comes from open tubules. Hydroxyapatite can help occlude them—here is the clinical evidence, timelines, and how that compares with other desensitising approaches.
Hydroxyapatite (HA) is among the better-supported toothpaste ingredients for dentin hypersensitivity—the sharp, short pain many people feel with cold drinks, cold air, sweet foods, or a blast of winter air on exposed roots. Unlike several other HA marketing themes, sensitivity has a concrete physical mechanism and a clinical literature that includes randomised trials and meta-analyses.

What “sensitive teeth” usually means
Healthy enamel is about 96–97% mineral by weight; dentin is roughly 70% mineral, with more organic matrix and water. Dentin contains microscopic channels called dentinal tubules. Under intact enamel and healthy gums, those tubules are not open to the mouth. When enamel thins, gums recede, or the surface is stripped by acid or aggressive brushing, tubules can open.
Fluid inside the tubules moves when temperature or osmotic changes hit the tooth. That movement stimulates nerves near the pulp. This is the hydrodynamic theory of dentin hypersensitivity—the mainstream explanation for the classic short, sharp “zing.”
Blocking the openings reduces fluid shift and, for many people, the pain. That is why occluding toothpastes, varnishes, and some professional treatments exist.
Sensitivity can also signal decay, a cracked tooth, a failing filling, sinus-referred pain, or gum infection. Persistent pain on one tooth, spontaneous throbbing, swelling, or pain that wakes you at night deserves a dental exam—not only a product swap.
How hydroxyapatite helps: tubule occlusion
Synthetic HA in toothpaste is a calcium phosphate mineral chemically similar to the mineral in enamel and dentin (biomimetic, not identical to imperfect biological apatite). Particles can adhere to exposed dentin and physically plug open tubules. Laboratory scanning electron microscopy (SEM) work repeatedly shows occluded tubule openings after HA exposure. How this differs from potassium nitrate
Many familiar “sensitive teeth” pastes (including popular Sensodyne-type formulas) rely heavily on potassium nitrate or related salts. Those mainly reduce nerve excitability. They do not primarily rebuild a mineral seal over tubule mouths.
HA’s approach is closer to other occluding strategies—certain calcium phosphates, bioactive glasses, and some arginine or oxalate systems—with the biomimetic angle that the occluding material resembles tooth mineral. Some people prefer that story; others do better with nerve-calming pastes. The mechanisms are different, so “which is better” is not a single universal ranking.

What the clinical evidence shows
Limeback, Enax & Meyer meta-analysis (2023)
A 2023 systematic review and meta-analysis in Biomimetics analysed clinical hypersensitivity trials of hydroxyapatite oral-care products. The systematic pool included a large set of trials (on the order of dozens in the broader analysis). Key pooled estimates for mean relative reduction:
- Versus placebo ≈ 39.5% (95% CI 30.06–48.93)
- Versus fluoride ≈ 23% (95% CI 11.82–34.18)
- Versus other desensitisers ≈ 10.2% (confidence interval crossed zero—a non-significant tendency)
Those numbers are averages across heterogeneous products and pain stimuli. They are still among the clearest quantitative summaries available for HA and sensitivity.
Earlier nano-HA meta-analysis (de Melo Alencar, 2019)
A 2019 meta-analysis focused on nano-hydroxyapatite: six randomised controlled trials with about four weeks of follow-up. It found better desensitising effects for evaporative stimuli (standardised mean difference about −1.09) and tactile stimuli (about −0.93) versus comparators. Cold-stimulus results were not statistically significant in that pool. The authors called for longer-term RCTs before definitive clinical guidelines.
That pattern—stronger on some stimuli than others, short follow-up—is typical of desensitiser research generally, not unique to HA.
How confident should you be?
On this site’s evidence map, sensitivity relief is a moderate-to-strong clinical claim for HA: coherent mechanism, multiple RCTs, and meta-analytic support. That is stronger than HA whitening evidence and clearer than most microbiome marketing claims.
Caveats still apply:

- Follow-up is often weeks, not years.

“Relatively strong for HA” is not “settled forever for every formula on every shelf.”

You do not have to pick a philosophy forever. Some people use an HA paste twice daily; others keep a potassium-nitrate paste for flares; some use professional varnishes for stubborn cases. Alternating without a plan makes it hard to tell what helped—give one approach a fair few weeks unless your dentist advises otherwise.
Realistic timelines
Most hypersensitivity trials evaluate change over about two to four weeks of regular brushing. Expect gradual quieting of triggers, not an instant anaesthetic effect like a clinic varnish or laser desensitisation.
A useful household experiment:
- Pick one HA toothpaste with a disclosed concentration.
- Use it twice daily for three to four weeks with a soft brush.
- Note which triggers change (cold air, ice water, sweet foods).
- If nothing improves—or pain worsens—stop self-experimenting and get examined.
Some people feel relief within days; others need the full trial window. Occlusion builds with repeated deposition and cleaning habits. One heroic brush will not remodel exposed roots.
Habits that help (and habits that undo the paste)
Technique often matters as much as the tube:

After whitening, orthodontics, or gum recession
Whitening: Peroxide can temporarily increase sensitivity. HA whitening itself is a weaker claim area—optical/surface effects, not bleach—so do not buy HA primarily as a peroxide substitute.

- Sudden and severe
- Localised to one tooth after trauma or biting something hard
- Accompanied by swelling, bad taste, fever, or visible broken tooth
- Unchanged after a few weeks of gentle care and an occluding paste
- Interfering with eating or sleep

Who is a good candidate for an HA sensitivity paste?
HA occlusion is a reasonable first-line home option when:

- You want a fluoride-free or biomimetic paste and understand the wider evidence trade-offs
- You are willing to brush gently twice daily for at least a few weeks
It is a weaker solo plan when:
- Pain is spontaneous, lingering, or wakes you at night (possible pulp involvement)
- One tooth hurts after biting something hard (possible crack)

Nano vs micro for sensitivity specifically
Marketing often implies nano-HA is automatically better at plugging tubules because particles are smaller. That size story is plausible: dentinal tubule diameters are on the micrometre scale at the surface, and nanoscale particles can physically lodge in openings.
Still, several well-known HA clinical programmes used microcrystalline HA, and hypersensitivity meta-analyses pool mixed product types. Do not reject a disclosed 10% micro paste solely because it is not labelled nano—and do not assume a “nano” badge guarantees clinical superiority without trial data on that exact formula. What success looks like (and what it does not)
Success usually means fewer zings, shorter zings, or needing less avoidance of cold drinks—not a permanently numb tooth. You should still feel normal temperature in a muted way. Complete loss of sensation is not the goal and can mask problems.
Failure after a fair trial does not mean “HA is fake.” It may mean the diagnosis was wrong, abrasion continues to reopen tubules, acid exposure is relentless, or you need a professional seal. Change the plan; do not spiral into paste-hopping every three days.
