How to evaluate any hydroxyapatite toothpaste—concentration, particle form, abrasivity, surfactants, humectants, flavour, sweeteners, and price per gram—without marketing fog.

Buying hydroxyapatite (HA) toothpaste should feel like reading a label, not decoding a vibe. Brands differ in concentration, particle size, abrasives, surfactants, flavour systems, and honesty about what the research actually tested.

Why label literacy matters

“Hydroxyapatite toothpaste” is not one thing. Stoichiometric microcrystalline HA, nano-HA rods, zinc-substituted carbonate HA, and undisclosed mystery powders can all appear under similar front-of-pack language. Concentrations in research often cluster around 5% and 10%, but retail labels sometimes hide the dose entirely.

If you cannot answer “how much HA, and in what form?” you cannot map the tube to the studies you see online. That is the core buying problem this page solves.

1. HA concentration (percentage)

What research suggests:

  • Huang et al. (2009, in vitro): remineralisation rose with nano-HA up to about 10%, with 10% ≈ 15% (plateau). Going far above that had limited incremental benefit in that model and raises formulation challenges (aggregation).
  • Najibfard et al. (2011, in situ): 5% and 10% nano-HA both remineralised early lesions; no significant difference versus each other in percent mineral gain in that protocol.
  • Schlagenhauf 2019 and Paszynska 2021/2023 caries non-inferiority RCTs: predominantly 10% microcrystalline HA.

Rule of thumb: Prefer disclosed 5–10% over mystery dosing. Do not assume a marketing paste with “maximum mineral” language beats a studied 10% formula.

2. Form disclosed: nano, micro, or blend

Ask: Does the label or site say nano, microcrystalline, or a blend? Any particle-size data?

  • Nano-HA usually means particles with at least one dimension in the nanometre range (often discussed as roughly 20–100 nm in oral-care literature; SCCS dossier examples include rod-shaped particles with median lengths around tens of nanometres).
  • Microcrystalline HA is larger; it is what several caries RCTs actually used.
  • Blends combine both.

EU safety opinions for nano-HA are morphology-specific (rod-shaped, aspect-ratio limits, uncoated; needle-shaped particles excluded from the safe conclusions). A “nano” badge without characterisation is incomplete for regulatory safety claims—see is hydroxyapatite toothpaste safe?.

3. RDA / abrasivity

Ask: Does the brand publish RDA (relative dentin abrasivity) or describe abrasives clearly?

HA itself is a mineral powder; the full paste’s polish system (silica grade, calcium carbonate, baking soda grade, charcoal, etc.) decides how hard it scrubs. For sensitivity, recession, or acid wear, lower abrasion is usually wiser. “Whitening” on the box sometimes means more scrub, not bleach.

If RDA is unpublished, treat aggressive charcoal blends and gritty powders as higher risk until proven otherwise. A biomimetic active inside a sandpaper vehicle is still sandpaper.

4. Surfactant: SLS or not?

Sodium lauryl sulfate (SLS) is a common foaming agent. Some people link it to mouth-ulcer irritation; evidence is mixed and individual. Many HA pastes skip SLS and use milder surfactants (for example, amino-acid–based cleansers such as sodium cocoyl glutamate).

Absence of SLS is a comfort preference, not evidence that the paste remineralises better. Judge HA percentage and abrasivity separately from the foam story. Foam is not cleaning power.

5. Humectant base

Toothpaste needs humectants so it does not dry into chalk in the tube. Common choices: sorbitol, glycerin, propylene glycol, and similar polyols.

You are looking for a stable, non-gritty paste that disperses on the brush and does not separate into watery layers. Exotic “oil-pulling toothpaste” textures can be hard to use consistently—consistency beats purity theatre. If you hate the mouthfeel, you will under-dose the active by skipping nights.

6. Flavour system

Peppermint, spearmint, and menthol dominate for a reason: people finish the tube. Kids’ pastes often use milder flavours. Some adults want “less toothpaste taste”; that is fine if the formula still discloses HA.

If a paste tastes medicinal or numbing in a way you hate, you will under-brush. Flavour is not a clinical endpoint, but it is a compliance endpoint—and compliance is where remineralising pastes live or die.

7. Sweetener

HA pastes often use xylitol, stevia, or sugar alcohols instead of sucrose (toothpaste should not be sugary). Xylitol has its own literature as a sugar substitute in caries ecology; it is a co-ingredient, not a replacement for evaluating HA dose.

Watch for marketing that treats xylitol as if it alone made a fluoride-free paste “clinically proven.” Judge each active on its own evidence.

8. Price per gram

Compare price ÷ net weight, not sticker shock alone.

  • Note multipacks and subscriptions separately from one-off tubes.
  • A cheap paste with 0% disclosed HA may be expensive in outcomes.
  • An expensive paste with clear 10% HA, published abrasivity, and a mild surfactant can still be rational.
  • Tiny “travel” sizes inflate $/g; compare like with like.

Retail channel (clinic, marketplace, brand site) changes price more than mineral physics does. Do the arithmetic once; it inoculates you against luxury packaging.

9. Claims that outrun the evidence

Walk away—or at least slow down—when you see:

  • “Prevents cavities as well as fluoride” stated as settled public-health fact (specific non-inferiority RCTs exist for certain 10% HA pastes; fluoride’s evidence base is still far larger)
  • “Rebuilds enamel” for large cavities that need fillings
  • “Whitens like professional bleaching”
  • “FDA-approved HA for caries” or “ADA Seal” for anticavity HA paste in the US sense (ADA Seal anticavity toothpastes contain fluoride)
  • “SCCS-approved nano” without rod / aspect-ratio / concentration caveats
  • “Balances your microbiome” as a clinical certainty from short anti-adhesion studies
  • “NASA invented this toothpaste” (NASA patented a crystal-growth concept; Sangi commercialised particulate HA paste)

Match claim strength to evidence strength. Sensitivity has relatively strong clinical support; whitening is weaker; caries data are promising non-inferiority trials with limitations; erosion protection is mostly pre-clinical.

Other reputable HA toothpastes exist

Japan has decades of commercial HA toothpaste history (Apadent launched 1980; Apagard 1985; anti-caries recognition for Sangi’s pathway in 1993). “Reputable” here means long use or published research ties—not a personal endorsement of every SKU. Apply the scorecard; do not outsource judgment to influencer rankings.

Co-ingredients: helpful, neutral, or noisy

Beyond HA itself, ingredient decks tell you how the paste will feel and whether marketing is stacking buzzwords.

  • Multiple remineralising actives listed without percentages (hard to know what you are paying for)
  • Peroxide in the same consumer toothpaste at cosmetic levels that will not match tray bleaching—yet still irritate some mouths

Substituted HA systems are not interchangeable with stoichiometric HA

Some European products use zinc-substituted carbonate hydroxyapatite (ZnCHA) at relatively high mineral loadings. Materials papers have discussed zinc-substituted HA weight percentages that look very high compared with a simple “10% HA” label. Those systems are chemically related but not identical to stoichiometric Ca₁₀(PO₄)₆(OH)₂ pastes, and percentages are not always comparable one-to-one.

When comparing, ask: stoichiometric HA, carbonate HA, zinc-HA, nano, micro, or blend? Then ask for the percentage that matches how the brand defines that ingredient. Blind “higher % wins” shopping fails across chemistries.

Reading study citations on product pages

Brands love citing papers. Apply three filters:

  1. Was the study on this exact product? Many citations are ingredient-class studies or competitor formulas at 10% microcrystalline HA.
  2. What was the endpoint? Sensitivity visual analogue scores are not the same as DMFS caries endpoints or shade-guide whitening units.
  3. Who funded it? Industry funding is common and disclosable; it is a reason for careful reading, not automatic dismissal.

If a product page cites Paszynska 2023 or Schlagenhauf 2019, remember those trials tested specific 10% microcrystalline HA pastes in defined populations with non-inferiority margins—not every HA SKU on the internet.

Regulatory and Seal reality checks (US-focused)

In the United States, fluoride is the OTC monograph anticaries active for toothpaste drug claims. HA pastes are generally sold as cosmetics (cleaning, appearance, comfort-oriented language). They typically will not carry an ADA Seal for cavity protection, because Seal anticavity toothpastes require fluoride.

That is a labelling fact, not a moral failing of HA. It does mean you should be suspicious of any HA tube that waves an anticavity Seal it cannot have, or implies FDA drug approval for caries prevention.

Worked example: comparing two imaginary tubes

Suppose Paste A costs more but lists 10% nano+micro HA, no SLS, published mild silica abrasive, mint, xylitol. Paste B is cheaper, says “with hydroxyapatite,” includes charcoal and SLS, and promises “peroxide-level whitening.”

Quick reference: minimum viable label

Before you pay, you should be able to fill in this sentence:

“This paste contains about ___% hydroxyapatite as a ___ (nano / micro / blend / substituted) form, uses ___ as the main surfactant, feels ___ abrasive, tastes like , and costs about $ per gram.”

If too many blanks remain after reading the box and the brand site, keep shopping. HA is not scarce anymore; disclosure still is.

Bottom line

The useful buying questions are boring on purpose: % HA, form disclosed, abrasivity, SLS, humectant base, flavour, sweetener, price per gram, and claim honesty. A paste that wins on those axes is easier to trust than one that wins on packaging.

What % HA should I look for?

Research commonly studies about 5% and 10%. In-vitro work often plateaus near 10%; an in-situ study found 5% and 10% nano-HA similar for mineral gain; major caries non-inferiority RCTs mostly used 10% microcrystalline HA. Prefer a disclosed percentage over a mystery “with hydroxyapatite” claim. More than 10% is not automatically better.

Is nano always better?

No. Nano particles are small enough to access microscopic defects and tubules, which is mechanistically appealing, but clinical superiority of nano over microcrystalline HA for cavities is not settled by large head-to-head caries RCTs. Several important caries trials used microcrystalline 10% HA.

What ingredients pair well with HA?

How do I spot marketing fluff?

Red flags include bleach-level whitening promises, “cures cavities,” “ADA Seal for HA anticavity,” “FDA-approved for caries,” “identical to natural enamel” without hedging, or “SCCS approved” without morphology details. Ask for % HA, particle form, and abrasivity data.

Does price equal quality?

Not reliably. Price tracks branding, tube size, retail channel, and sometimes particle grade—but an expensive paste with undisclosed % HA can be worse value than a cheaper paste that publishes concentration and RDA. Compare price per gram and label clarity together.
